This post does not argue against GLP-1 medications. Anyone using Ozempic or a similar drug and experiencing relief is experiencing something real. The biology here is about what the drug does and does not address — not about whether the relief is valid. Ozempic works. For most people who use it, the experience is of something finally working after years of nothing working. That relief is real and should not be diminished.

And for most people who stop using it, the weight comes back. Not all of it, immediately — but 65 to 75 percent within a year of stopping, on average. That is also real. And it is not a patient failure. It is a mechanistic consequence of what the drug does and does not change at the biological level.

What Ozempic Actually Does

GLP-1 receptor agonists work by mimicking glucagon-like peptide-1, a hormone naturally released by the gut after eating. They signal the pancreas to release insulin, slow gastric emptying, and send signals to the hypothalamus that reduce appetite — producing average weight loss of 15–22% of body weight in trials with meaningful improvements in blood glucose control. This is a signal intervention. It changes what a message says without changing the infrastructure the message travels through. The drug tells the hypothalamus to reduce appetite. It does not change the lipid composition of the cell membranes the hypothalamus is built from.

What Ozempic Changes vs. What It Leaves Untouched

Why the Weight Returns: The Substrate Problem

CB1 over-activation in adipose tissue is one of the primary mechanisms of visceral fat accumulation. In a membrane built from an 18:1 omega-6 to omega-3 ratio, CB1 in fat tissue is structurally predisposed to over-activation — the receptor environment is pro-inflammatory and dysregulated at the substrate level. GLP-1 agonists do not change this. When the drug stops, the regulatory environment the body has been operating in — depleted ECS, dysregulated CB1 in adipose, inadequate anandamide production for honest satiety signaling — is still exactly what it was.

The cortisol-glucocorticoid loop is the other missing piece. Chronic cortisol elevation drives visceral fat accumulation through glucocorticoid receptor activation in adipose tissue, independently of caloric intake. This is the mechanism that most weight frameworks — including the framework Ozempic operates within — don't address. Dietary cortisol management, through glycemic stability and substrate restoration, speaks directly to this loop.
